TERMS AND CONDITIONS OF USE

Ann & Robert H. Lurie Children's Hospital of Chicago and its affiliates ("Lurie Children's") and/or your independent physician practice are pleased to offer you medical information via a Web-based version of our computer systems called MyChart. MyChart is not a complete medical record. MyChart only includes selected ambulatory care records and does not provide access to inpatient records.

We think it is important for you to know how we handle information we communicate via the Internet. This Terms & Conditions statement outlines our practices and our sensitivity to your right to privacy. We reserve the right to revoke access at any time for any reason.

Account Holder

An individual must be age 18 or older to become an account holder. Parents or legal guardians of children under 18 years old may become an account holder via a request for proxy access to their child's account by submitting required information and the appropriate MyChart authorization form(s) to the contact information listed in the form. Minor children between the ages of 12 and 17 (inclusive) will not be eligible to participate in MyChart services, but must provide consent for proxy access to their medical information. When a minor patient reaches age 18, MyChart access to the proxy account holder will be automatically revoked, and the patient may then set up his or her own account. If the patient is 18 or older and wishes for a parent or guardian to become or remain a proxy account holder with joint access, the patient and proxy must complete a MyChart authorization and consent form and submit it to the contact information listed in the form.

Response to Electronic Communication

Electronic communication through MyChart is intended for requesting an appointment, requesting a non-urgent medication refill and non-urgent questions. Specific questions regarding care and treatment of the patient will not be processed or responded to electronically. If you have a medical emergency or have a question about the care or treatment of a patient, please do not use the e-mail communication options provided through MyChart. Instead, contact your healthcare professional directly. This site is not a substitute for appropriate and timely contact with your clinician.

Lurie Children's and/or your independent physician practice will use best efforts to provide a timely response to electronic inquiries. In some cases, the clinic staff that needs to respond to an electronic inquiry or other communication may not be immediately available. A patient should allow at least 48 hours to receive a response if the message was received during our hours of operation (Monday-Friday: 8am-4pm). Emails received after those hours, or over weekends and holidays will be considered received the next business day and responded to within 48 hours.

MyChart may offer links to related medical Web sites not managed by Lurie Children's or your independent physician practice. These Web site link(s) are for a patient's informational purposes only. Neither Lurie Children's nor your independent physician practices endorse or has verified the accuracy of the information in/on these Web sites, and the patient should not rely on any of the information found on the Web sites for purposes of treatment or diagnosis.

E-mail Privacy

MyChart users should be aware that they will be notified via e-mail when there is new medical information to be viewed on MyChart. This means that any person with access to the e-mail account provided to Lurie Children's and/or your independent physician practice will be able to see this notification. Although no private medical information will be sent, the notification that new medical information is available by accessing MyChart may be information that a patient would not want others to know. Thus, patients should take this into account when providing an e-mail address.

Please know that if you send an e-mail communication, it may be shared with the Lurie Children's and/or your independent physician practice staff that assists the physician(s) in providing the patient's medical care.

Security and Confidentiality

Lurie Children's and your independent physician practice are committed to protecting the confidentiality of your medical information. We afford the same degree of confidentiality to medical information stored in MyChart as is given to medical information stored by Lurie Children's in any other medium.

[For other than general information viewing, MyChart must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal (e.x. Firefox, Chrome, or Internet Explorer versions 8.0 or greater). Our SSL web server uses authentication and offers the highest level of encryption technology commercially available (128-bit RC4).

You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means your data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to Lurie Children's by viewing the digital certificate. This certificate verifies the connection between the Lurie Children's server's public key and the server's identification.]

Information Does Not Constitute Medical Advice

Lurie Children's and/or your independent physician practice assume no responsibility for how you use the information you obtain from this site.  You should consult with a physician or other healthcare professional regarding the patient's condition. 

Please contact your physician directly and do not utilize the e-mail communications in MyChart if:

  • You wish to communicate with one of your healthcare professionals about information contained in MyChart or pertaining to clinical care and treatment; or
  • You have an emergency or believe you need to speak to someone urgently.

Neither MyChart nor any other application available through this site is a substitute for appropriate and timely contact with your physician. This site is not a substitute for consultation with your physician. You should never change or stop any course of treatment prescribed by your physician without first consulting him or her.

Termination of Access

Lurie Children's and/or your independent physician practice may restrict or terminate access to MyChart at any time. If access is restricted for planned maintenance of the MyChart system, every effort will be made to communicate downtime to all users in advance. However, there may be reasons to restrict access to MyChart without advance notice. If your use of MyChart violates these Terms and Conditions, Lurie Children's and/or your independent physician practice reserve the right to permanently terminate access to MyChart.
