Ann & Robert H. Lurie Children's Hospital of Chicago and its affiliates ("Lurie Children's") and/or your independent physician practice (and together are referred to in this statement as “Provider”) are pleased to offer you medical information via a Web-based version of our computer systems called MyChart. MyChart is not a complete medical record. MyChart only includes selected portions of the medical record.

This Terms and Conditions of Use statement outlines Provider's practices and important guidelines of use of your MyChart account. You agree to these Terms and Conditions of Use by using MyChart. Provider reserves the right to modify these Terms and Conditions of Use at any time.


MyChart Account Holder

In general, an individual must be age 18 or older to become an account holder. Lurie Children's reserves the right to revoke MyChart access at any time and for any reason, including compliance with applicable law.

Patients aged 12-17: Under State and Federal law, there are certain types of medical information that the parent or guardian of a minor patient age 12 or older may not view without consent of the minor patient. Because of these requirements, when a minor patient reaches age 12, we require their consent before access to their MyChart account can be provided. In general, minor patients between the ages of 12 and 17 (inclusive) will not be eligible to be an account holder. Provider reserves the right to grant special requests, in its sole and absolute determination.

Patients over the age of 18: When a minor patient reaches age 18, MyChart access to a proxy account holder will be automatically revoked, and the patient may then set up an account. If the patient is 18 or older and wishes for a parent or guardian to become or remain a proxy account holder with joint access, the patient and proxy must complete a MyChart authorization and consent form and submit it to the contact information listed in the form.

Proxy Access Requests: Individuals age 18 or older may request proxy access to another individual's MyChart account by completing the Proxy Access MyChart Consent Form and submitting it to Provider. Such access will only be granted to parties with the express consent of the patient, and/or with parental rights or legal guardianship over the MyChart account holder and only to the extent that the party requesting proxy access can demonstrate the legal right to account holder's medical information. If the proxy's legal relationship with the account holder changes, the proxy must inform the Provider immediately.


Response to Electronic Communication

Electronic communication through MyChart is intended for routine medical questions, such as requesting an appointment, requesting a non-urgent medication refill and non-urgent questions. Specific questions regarding care and treatment of the patient will not be processed or responded to electronically. This site is not a substitute for appropriate and timely contact with your Provider.

For all urgent medical matters, contact your provider office directly, go to an emergency room, or call 911.

Provider will use best efforts to provide a timely response to electronic inquiries. In some cases, the staff that needs to respond to an electronic inquiry or other communication may not be immediately available. A patient should allow at least 48 hours to receive a response if the message was received during our hours of operation (Monday-Friday: 8am-4pm). Emails received after those hours, or over weekends and holidays will be considered received the next business day and responded to within 48 hours.


Photographs/Digital Images Uploaded by Patients

If you upload any photograph or digital image to MyChart, you represent that, including without limitation, the photograph or digital image:

  • Is of the patient and not of anyone else; and
  • Is a true and accurate representation of your health status to best of your ability; and
  • Has not been misrepresented in any way; and
  • That you have unlimited rights to the photograph or digital image and it is not subject to any copyright or trademark; and

You must understand and acknowledge that any communication you receive in response to the photograph or digital image is based upon what Provider is able see at the time it is viewed. Provider does not have any control over the clarity of how photographs or digital images are taken, uploaded, transmitted or ultimately appear on any computer or mobile device. Any advice or recommendation you may receive from Provider based on submission of this photograph or digital image, is not a substitute for an examination and/or an office visit. Do not wait to seek immediate attention if you feel your condition is worsening or if it is different than what the provider is able to discern from the photograph or digital image.

Provider has the right to remove any photo or digital image that it determines in its sole and absolute discretion is inappropriate, offensive, in violation of these Terms and Conditions of Use, in violation of any state or federal law, and/or does not otherwise belong in MyChart.


Web Site Links

MyChart may offer links to related medical Web sites not managed by Provider. These Web site link(s) are for a patient's informational purposes only. Provider did not endorse or verify the accuracy of the information in/on these Web sites, and the patient should not rely on any of the information found on the Web sites for purposes of treatment or diagnosis.


E-mail Privacy

MyChart users should be aware that they will be notified via e-mail when there is new medical information to be viewed on MyChart. This means that any person with access to the e-mail account given to Provider will be able to see this notification. Although no private medical information will be sent, the notification that new medical information is available by accessing MyChart may be information that a patient would not want others to know. Thus, patients should take this into account when providing an e-mail address.

Please know that if you send an e-mail communication, it may be shared with Provider's staff that assists the physician(s) in providing the patient's medical care.


Security and Confidentiality

Provider is committed to protecting the confidentiality of your medical information. We afford the same degree of confidentiality to medical information stored in MyChart as is given to medical information stored by Provider in any other medium.

[For other than general information viewing, MyChart must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal (e.x. Firefox, Chrome, or Internet Explorer versions 11.0 or greater). Our SSL web server uses authentication and offers the highest level of encryption technology commercially available.

You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means your data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected securely by viewing the digital certificate. This certificate verifies the connection between the Lurie Children's server's public key, or your independent physician practice's server's public key, and the server's identification.]


Information Does Not Constitute Medical Advice

Provider assumes no responsibility for how you use the information you obtain from this site. You should consult with a physician or other healthcare professional regarding the patient's condition.

Please contact your Provider directly and do not utilize the e-mail communications in MyChart if:

  • You wish to communicate with one of your healthcare professionals about information contained in MyChart or pertaining to clinical care and treatment; or
  • You have an emergency or believe you need to speak to someone urgently.

Neither MyChart nor any other application available through this site is a substitute for appropriate and timely contact with your physician. This site is not a substitute for consultation with your physician. You should never change or stop any course of treatment prescribed by your physician without first consulting your physician.

All content included on Lurie Children's websites, including, but not limited to, text, photographs, graphics, button icons, images, artwork, names, logos, trademarks, service marks and data (the "Content"), in any form including the compilation thereof, are protected by U.S. and international copyright law and conventions. The Content includes both Content owned or controlled by Lurie Children's and Content owned or controlled by third parties and licensed to Lurie Children's. Except as set forth below, direct or indirect reproduction of the Content, in whole or in part, by any means, is prohibited without the express written consent of Lurie Children's.

Site Access and Licenses

Lurie Children's grants a limited license to each user to make personal use only of the Web site and the associated services in accordance with these Terms and Conditions of Use. This license expressly excludes, without limitation, any reproduction, duplication, sale, resale or other commercial use of the Web site and the associated services, making any derivative of the Web site or the associated services, the collection and use of user email addresses or other user information, including, without limitation, health information or any data extraction or data mining whatsoever. Through this License, you may be able to upload third party data into MyChart or download limited medical information from MyChart to select third party sites.


No Warranties

This site and all other sites hosted by Lurie Children's and the content contained herein and therein are provided by Lurie Children's is on an "as-is" basis. Lurie Children's makes no representations or warranties of any kind express or implied as to the operation of its sites, or the content, products and/or services included therein. To the fullest extent permissible by applicable law, Lurie Children's disclaims all warranties, express, or implied, including but not limited to implied warranties of merchantability, fitness for a particular purpose, title, and/or infringement.


Limitation of Liability

Some of the material on the Lurie Children's site is provided by third parties and Lurie Children's shall not be held responsible for any such third-party material. Lurie Children's disclaims any responsibility for or liability related to such third-party material. Any questions complaints or claims related to any product should be directed to the appropriate third party.


Termination of Access

Provider may restrict or terminate access to MyChart at any time. If access is restricted for planned maintenance of the MyChart system, every effort will be made to communicate downtime to all users in advance. However, there may be reasons to restrict access to MyChart without advance notice. If your use of MyChart violates these Terms and Conditions of Use, Provider reserves the right to permanently terminate access to MyChart.
